Insurance Verification & Estimated Coverage

If you are using an insurance plan I participate with, benefits will be verified and a cost estimate provided before your first visit. Insurance verification is an estimate only and does not guarantee coverage or final cost. Copays, deductibles, and coinsurance are determined after the claim is processed. You are responsible for any portion not covered by insurance at the time of service. In the event of a discrepancy between the estimated insurance benefits and the final processed claim, we will follow up with your insurance provider to clarify the issue and determine the appropriate charge. If the final amount differs from the estimate, you will be notified before invoicing/billing.

Verification takes up to one week and is for those seriously considering working together.

You may also check it yourself/if you you are using an out of network plan: Call your insurance using the sample script. I can help with ICD-10 codes—email dsnutritionpllc@gmail.com.

Common Coverage Scenarios

1199SEIU:

  • Solid coverage for nutrition services.

Cigna:

  • Often covers up to 3 visits, with the potential for more if there is a medical diagnosis such as PCOS, diabetes, high cholesterol, or a BMI-related condition.

Note: While insurance may require BMI as a qualifier, I do not use BMI as a primary health indicator and recognize the limitations and potential harm in focusing solely on BMI or weight.

Aetna:

  • Generally good coverage but often involves copays and deductibles.

  • Can be the most challenging to verify coverage with.

  • Coverage examples:

    • Up to 10 visits for “Healthy Diet Counseling”

    • Up to 26 visits for “Obesity Counseling”

MetroPlus:

  • Usually provides solid coverage for nutrition services.

Out-of-Network Coverage

If I am not in-network with your insurance, I can provide a superbill upon request. Some out-of-network plans (e.g., UHC and Emblem) may still reimburse for nutrition services, depending on your benefits.
