Billing and Insurance Coverage POLICY

I am in network with American Specialty Health, Aetna, Cigna, MetroPlus and 1199SEIU, however they decide which specific plans I can accept and receive reimbursement from. Depending on the patient and insurance plan, we may need to seek a referral or documentation for a specific diagnosis code (ICD-10 code) in order for me to submit a reimbursable insurance claim. If this is the case, the insurance provider will likely cover a medical diagnosis of PCOS, diabetes, high cholesterol, hypertension, digestive disorders, eating disorders, BMI > 30 *, etc. I will help with this to the best of my ability and we will discuss our options prior to our first session together. 

If you are interested in using insurance, the billing company I work with will verify with your insurance provider what is covered and the estimated cost to you before your complimentary call, initial session or during follow up sessions (if your insurance only covers a specific number of sessions per year).** I have noticed some clients with insurance companies I am out of network with are having my services covered. This is why no matter what insurance you have, I suggest we check your benefits. For out of network clients, a superbill will be provided upon request.

*For insurance purposes only - I have not found it useful to use BMI as a health indicator and problematic to focus solely on BMI and/or weight.

**I work closely with a billing company to verify your benefits and cost estimation as best as possible, however we cannot 100% confirm the cost to you until the insurance claim is processed, unfortunately. There are times we are told one thing, but then something changes as the claim is processed despite our best efforts upfront. We are constantly striving to provide the best estimation.

Cancellation Policy

I specifically reserve time for your nutrition appointment. If you need to cancel or reschedule your appointment, please do so by contacting me (Danielle Sobieski MS, RDN, CDN) via e-mail or phone with a minimum of 24 hours notice from your appointment time. Appointments not canceled within this timeframe will be charged a flat fee of $50 dollars. Canceled sessions may be rescheduled within the same week, at no additional fee, when possible. Canceled sessions cannot be billed or submitted to insurance for reimbursement. 

LATENESS Policy

Appointments will be held for 15 minutes only. If you arrive within this time period, you will be seen but your appointment time will not be extended. After 15 minutes, your appointment will be forfeited, and you will be charged a flat fee of $50 dollars. The fee will be waived if the appointment is rescheduled within the same week, when possible.
